Appointments: Please arrive 10 minutes prior to your appointment time to get settled and prepare for your massage. Clients that are late for their appointment will have their session shortened to ensure subsequent clients are not affected. Clients must arrive with good hygiene and wearing appropriate attire. Although same day appointments are available at times, it is recommended that you reserve your appointment a few days in advance.
Payments: I accept cash and personal checks. There is a $25 fee for all returned checks.
Gift Certificates: Gift Certificates are non-refundable and must be present at the time of service. They cannot be replaced if lost or stolen.
Packages: May be transferred to another person upon approval. All requests must be submitted in writing (email is ok).
Traveling Fee: There is a $20 traveling fee for all destinations. Gratuities: Gratuities are never required and are always appreciated. Please feel free to tip as you see fit. Cancellations and No-Shows: I require a minimum 24 hour notice for cancellations and rescheduling for all appointments to avoid being charged. Clients that are unavailable at their appointment time, or No-Show, are responsible for full payment.
Therapist Responsibilities:
Professionalism: You will experience a professional session each time and will always be treated with respect and dignity. Personal and professional boundaries are respected at all times. Every session will utilize full discreet draping. Respect will be demonstrated by honoring client's process, being present, listening, asking only pertinent questions, draping properly and maintaining professionalism.
Communication: Gathering feedback from you during every session will be ongoing so I may monitor your confort level and strive to give you the best experience possible. Each session will be documented and I respect the confidentiality of all personal and health information. I follow in accordance with HIPPA regulations.
Ethics: Respect will be shown to all clients regardless of their age, gender, race, national origin, sexual orientation, religion, socio-economic status, body type, political affiliation, state of health and personal habits. I will perform only services for which I am qualified and able to do, and refer to appropriate specialists when work is not within my scope of practice or not in the client's best interest. All fees charged will be fair. All city, county, state and national laws and regulations will be adhered to.
Termination of Session: I do professional massage and bodywork only. Inappropriate language and actions will result in termination of your session with the scheduled payment due in full. I reserve the right to refuse service.

It is my choice to receive massage therapy. I realize that the treatment is being given for the well-being of my body and mind. This includes stress reduction, relief from muscular tension, spasm or pain, or for increasing circulation or energy flow. I agree to communicate with my massage practitioner any time I feel that my well-being is being compromised.
If I experience any pain or discomfort during my session, I will immediately inform my practitioner so that the pressure/strokes may be adjusted to my level of comfort. I understand that massage practitioners do not diagnose illness, disease, or any physical or mental disorder; nor do they prescribe medical treatment, pharmaceuticals, or perform spinal thrust manipulations. I acknowledge that massage is not a substitute for medical examination or diagnosis, and that it is recommended that I see a primary health care provider for that service.
Because massage/bodywork should not be performed under certain medical conditions, I affirm that I have stated all my known medical conditions, and answered all questions honestly. I agree to keep my practitioner updated as to any changes in my medical profile and understand that there should be no liability on my practitioners part should I fail to do so. I also understand that any inappropriate actions or language will result in immediate termination of the sessions, and I will be liable for full payment of the scheduled services. All payments are due in full at the time of service. We reserve the right to refuse service to anyone.
